Abstract
Nyeri persalinan pada primigravida kala I fase aktif merupakan respons fisiologis akibat kontraksi uterus dan dilatasi serviks yang bersifat subjektif serta dipengaruhi oleh ambang nyeri individu. Manajemen nyeri nonfarmakologis, seperti kombinasi kompres hangat dan counter pressure massage, diperlukan untuk meningkatkan kenyamanan ibu secara maksimal tanpa menimbulkan efek samping pada janin. Penelitian ini bertujuan untuk mendeskripsikan asuhan keperawatan maternitas pada ibu primigravida dengan nyeri melahirkan melalui penerapan kombinasi kompres hangat dan counter pressure massage berbasis teori gate control. Penelitian ini menggunakan desain studi kasus keperawatan pada Ny. M (G1P0A0, usia kehamilan 36 minggu) dalam persalinan kala I fase aktif dengan tingkat nyeri awal berada pada skala Numeric Rating Scale (NRS) 7/10. Intervensi yang diberikan meliputi kompres hangat (suhu 38–40°C) pada area suprapubik dan lumbosakral serta counter pressure massage pada sakrum (S2–S4) selama kontraksi, yang dipadukan dengan teknik napas dalam dan pengelolaan lingkungan. Setelah pemberian intervensi selama 60 menit, skala nyeri NRS mengalami penurunan dari 7 menjadi 6, serta klien menunjukkan tanda-tanda rileks dan mampu mengontrol pola napas. Meskipun skala NRS kembali naik menjadi 7 pada fase transisi akibat puncak intensitas kontraksi, tingkat nyeri pascapersalinan (afterpain) menurun hingga skala 5. Klien mampu mendemonstrasikan ulang teknik yang diajarkan dan melaporkan tingkat kenyamanan yang lebih tinggi. Kombinasi kompres hangat dan counter pressure massage efektif meredakan intensitas nyeri persalinan pada primigravida melalui mekanisme blokade impuls nyeri (gate control theory) dan stimulasi pelepasan endorfin. Intervensi ini terbukti aman, aplikatif, serta sangat direkomendasikan sebagai terapi nonfarmakologis lini pertama dalam penanganan nyeri persalinan kala I fase aktif.

Labor pain during the active first stage in primigravida mothers is a physiological response resulting from uterine contractions and cervical dilation, which is highly subjective and influenced by individual pain thresholds. Non-pharmacological pain management, such as the combination of warm compresses and counter pressure massage, is essential to maximize maternal comfort without inducing adverse effects on the fetus. This study aims to report maternity nursing care in a primigravida patient experiencing labor pain through the application of combined warm compress and counter pressure massage based on the gate control theory. A nursing case study design was conducted on Mrs. M (G1P0A0, 36 weeks gestation) during the active first stage of labor presenting with an initial Numeric Rating Scale (NRS) pain score of 7/10. The intervention involved applying warm compresses (38–40°C) to the suprapubic and lumbosacral areas alongside counter pressure massage over the sacrum (S2–S4) during contractions, complemented by deep breathing exercises and environmental control. Following a 60-minute intervention, the pain score decreased from 7 to 6, with the patient appearing significantly more relaxed and capable of controlling her breathing. Although the NRS score rose back to 7 during the transition phase due to peak contraction intensity, it subsequently dropped to 5 post-delivery (afterpain). The patient successfully redemonstrated the technique and expressed greater physical comfort. Combining warm compresses with counter pressure massage effectively reduces labor pain intensity in primigravida patients through gate control mechanism and endorphin release. This intervention is safe, practical, and highly recommended as a first-line non-pharmacological nursing therapy during the active first stage of labor.
