Abstract

Background: Type 2 Diabetes Mellitus (DM) is a chronic metabolic disorder characterized by hyperglycemia due to insulin resistance. Chronic hyperglycemia carries the risk of peripheral neuropathy and reduced blood circulation to the lower extremities, thereby increasing the risk of impaired skin integrity leading to diabetic foot ulcers. Foot care therapy is an effective preventive nursing intervention to maintain skin moisture, enhance peripheral circulation, and prevent diabetic foot complications. Objective: To identify and evaluate the implementation of foot care therapy in Type 2 DM patients with the nursing problem of risk for impaired skin integrity in the Lower Teratai Ward of RSUD R.T. Notopuro Sidoarjo. Methods: This study utilized a descriptive case study design with a nursing process approach. The subject was Mr. N (51 years old) diagnosed with Type 2 DM with Hyperglycemia. Foot care therapy was administered for 3 consecutive days ($3 \times 24$ hours), involving foot washing with warm water (37–39°C), drying between toes, nail trimming, moisturizer application, simple foot exercises, and self-care education. Results: Prior to foot care therapy, the patient's right foot skin appeared dry, acral areas felt cold, with complaints of numbness/tingling, decreased peripheral circulation, an initial random blood glucose of 510 mg/dL, and HbA1c of 13.9%. Following the 3-day intervention, there was a noticeable increase in skin moisture, cleaner skin condition, reduced numbness and cold sensation, no new lesions/ulcers formed on the right foot, and the patient demonstrated the ability to perform independent foot care. Conclusion: Systematic and regular implementation of foot care therapy is effective in maintaining skin integrity, improving peripheral circulation, and preventing new diabetic foot lesions in Type 2 DM patients at risk for impaired skin integrity. Keywords: Type 2 Diabetes Mellitus, Foot Care Therapy, Risk for Impaired Skin Integrity, Peripheral Circulation, Diabetic Foot Care.
